Starting and Ending Your Utility Services

HOA ?  Before signing up for Utility Service, please check with your Home Owner's Association to see which services may be included in your HOA fees.

Signing Up For Service:

Welcome to Midvale City. If you have recently purchased a property in Midvale, you must fill out a Municipal Utility Services Agreement Form and provide a copy of the first page of your settlement statement from your closing documents to open your account.

There are two ways you can do this:

In Person at the Midvale City Hall:

 Midvale, UT 84047

 

Once you have submitted the completed service agreement and settlement page to our office we will open your new account. Your service will be billed from the date you purchased your property. 

 

Selling Your Property (terminating your service):

If you have recently sold your property, you must fill out a Termination of Service Form and provide a copy of the first page of your settlement statement from your closing documents to close your account.

There are two ways you can do this:

Once you have filled out the form and provide a copy of the first page of your settlement statement to our office we will close your account.  This service will now be billed to the new owner from the date you sold your property. 

*If a refund is due, Midvale City will send this to you by mail to the forwarding address you provided in the Termination of Service form.  Please allow four to six weeks for your refund to be processed.
